  I am requesting an investigation by the Oregon Medical Board (OMB) regarding the following situations:

1) Falsification of my case record (A#144783) by deletion of my sworn testimony. Once you recognize this falsification, I hope you will agree it was unreasonable to use a fake record to convict me, as both the OMB and the Oregon Court of Appeals did.
Was the falsification of the case record ordered by a Board Member? Did any Board Member approve the falsification? I believe the Board Members were unaware they were using a fake record.

2) The OMB never did issue a decision regarding one of the complaints filed against me. Why not? A different complaint was decided despite the lack of any contested case hearing regarding it or testimony regarding it. I actually never received the complaint and learned about it in the Final Order.
I doubt any jurist was aware there had been a switcheroo.

3) Repeated lies and fabrications by OMB personnel. Were these individuals ordered to lie? Examples are the claim that I had insisted I deserved an active license- the truth is that I did NOT file any request to reactivate the license. The complaint against me made up an arrest charge instead of using the one on the police report, and claimed I had been arrested for a charge that was created later, unrelated to the arrest (and contrary to sworn testimony by the arresting officers).

  The difficulty (impossibility) I have had trying to inform the OMB of irregularities; and the lack of any investigation in the 15 years since my involvement started-- is actually a sort of proof that Board Members are insulated from truth and very susceptible to fabrications created by OMB personnel. You might find there has been a "cover-up".

The truth was mostly preserved in OMB files, unless they have been altered since a copy was provided for appeal (in which case the Appeal Court should have a copy of the falsified OMB case record.
I believe that the falsification of the case record has been recognized by several lawyers, but none of them seem to have reported it to the jurists who used the faked records to make decisions.
