  I am requesting an investigation by the Oregon Medical Board (OMB) regarding the following situations:

1) Falsification of the case record A#144783- this falsified case record was used by the OMB and the Oregon Court of Appeals to make legal decisions. Was the falsification of the case record ordered by a Board Member, or did any Board Member approve the action.

2) The OMB never did issue a decision regarding one of the complaints filed against me. Why not?

3) I was not provided/offered a contested case hearing regarding a complaint that appeared in the final order of the OMB, but had never been issued by the OMB. Did the OMB jurists even know about this?

4) Repeated lies and fabrications by OMB personnel. Were these individuals ordered to lie?

  The difficulty (impossibility) I have had trying to inform the OMB of irregularities; and the lack of any investigation in the 15 years since my involvement started-- is actually the equivalent of an investigation that shows the Board Members are insulated from truth and very susceptible to fabrications created by OMB personnel.

The truth is mostly preserved in OMB files, but because these are mostly secret and considered "closed cases", nobody actually has looked at them to learn that all of my allegations are true and already proven, but the proof was concealed and/or ignored.
